Prices are effective as of January 1, 2001 and are subject to change without notice.

PAYMENT POLICY:
· Payment for goods and services is expected in full at the time of arrangements. For your convenience we accept cash, check, credit card (Visa, MasterCard, Discover & American Express).

Certified Death Certificates, Transportation, Sales Tax
The goods and services shown are those we can provide to our customers. You may choose only the items you desire. However, any cremation arrangements you select will include a charge for our basic services and overhead.
CREMATORY SERVICE POLICY:
Veterinary Fees

Saint Francis Pet Crematory will not pay veterinary fees or charges.
Basic Professional Services of Crematory Director and Staff
This is the basic charge to each family we serve for our professional expertise in assisting them. It includes, but is not limited to, a proportionate share of the taxes, licenses, utilities and business expenses necessary to serve the public in a professional manner. Also included in this charge, are the clerical and administrative services, preparation and filing of necessary permits required in by the Illinois Companion Animal Cremation Act.

Companion Animal Care and Grooming
All companion animals designated for private viewing will receive professional care and grooming.

Washing and disinfecting of companion animal remains.
Care and custody while sheltering remains.
Refrigeration of companion animal remains.

Directing Services and Use of Facilities
The use of the crematorium facility for visitation and services of crematory director and staff in supervising visitation are as follows:

Removal and Transfer of remains to crematory facility.
Limousine/Other Passenger Vehicle and Driver.
Utility vehicle and driver.
Flowers and props.

Other Merchandise and Supplemental Service
Urns/memorials
Memorial Print Package (Register book, Acknowledgement cards, pet loss book).
Cremains storage fee. Six (6) month maximum.
Scattering cremains at Lake Michigan , IL .
Rush Cremation Process Fee (Priority Handling)
Extra mailing receptacle for cremains.
Veterinary examiner/autopsy
Witness Cremation.
Shipping/air tray (required by airlines for shipment)

The only warranty of the casket or any merchandise sold in connection with this service is the express written warranty, if any, granted by the manufacturer. Saint Francis Pet Crematory makes no warranty, expressed or implied warranty of fitness for a particular purpose, with respect to the urn and/or merchandise and supplemental services.

We offer many different styles and prices of urns and alternative containers from which to select. These urns are not designed by the manufacturer to resist the entrance of air, water or other outside elements. These urns may be constructed of metal, hardwoods, or other wood products with varying gauges and weights.
